The Initial Consultation: Understanding the Patient’s Desires and Needs

Our patient, a gentleman in his early 30s, came to our office with a beautiful but imperfect smile. He had recently finished his Invisalign treatment, successfully straightening his teeth. However, he was concerned about the yellowish hue of his teeth and wanted to enhance their appearance without resorting to unnaturally white veneers. He desired a natural-looking smile that exuded confidence and elegance.

During his initial consultation, we conducted a thorough examination and discussed his goals. He expressed a preference for veneers that were not opaque white and looked natural. Additionally, he had canted teeth, which required levelling, and his anterior six teeth were hitting each other in a traumatic way, causing bone resorption and mobility. It was clear that we needed to address both the aesthetics and functionality of his smile.

The Treatment Plan: A Comprehensive Approach to Smile Transformation

Creating a treatment plan for this patient involved a multi-faceted approach, addressing both cosmetic and functional aspects. Here’s a detailed breakdown of the procedures we followed:

  1. Balancing the Canting with Porcelain Veneers:

    • The first step was to balance the canted teeth and level his smile using porcelain veneers. Porcelain veneers are a fantastic option for achieving a harmonious and symmetrical smile. These thin shells of porcelain are custom-made to fit over the front surfaces of teeth, providing a natural look while correcting various imperfections.

  2. Addressing Traumatic Occlusion:

    • The traumatic occlusion of the anterior six teeth needed immediate attention. To prevent further bone resorption and mobility, we decided to increase the length of the anterior teeth and adjust the vertical dimension of occlusion (VDO) using dental onlays and crowns on the molar teeth. If the VDO was not adjusted, increasing the length of the anterior teeth alone could create a deep bite, leading to more serious issues in the future.

  3. Designing the Smile in 2D and Creating a 3D Smile Prototype:

    • After finalizing the treatment plan, we designed the smile in 2D and sent the data to our dental lab. The lab then transferred the design to a 3D Smile Prototype, which allowed us to visualize the final outcome. This step is crucial as it ensures precision and accuracy in the smile makeover process.

The Mock-Up Process: A Sneak Peek into the Future Smile

The 3D Smile Prototype was printed and used to create temporary materials for a mock-up. This process has several advantages for the patient:

  • Visualization:

    • The patient could see and feel the proposed changes in real-time, providing a clear understanding of the expected results. This visualization helps build confidence and ensures the patient’s expectations align with the outcome.

  • Non-Invasive:

    • Importantly, this step is non-invasive. We do not shave or harm any tooth structure during the mock-up process. It is a safe and reversible way to preview the final results.

​

  • Feedback:

    • The patient’s feedback during the mock-up phase is invaluable. It allows for adjustments and refinements, ensuring complete satisfaction before proceeding with permanent restorations.

Our patient was thrilled with the mock-up and gave us his approval to move forward with the treatment.

The Kois Deprogrammer: Finding the Ideal Bite

To address the issues caused by years of grinding and clenching, we introduced the Kois Deprogrammer. This device helps identify the ideal bite that is comfortable and neutral for the muscles and brain. By wearing the Kois Deprogrammer for two weeks, the patient could find a bite that relieved stress on his teeth and muscles, providing a stable foundation for the subsequent treatments.

Preparation for Porcelain Veneers: Precision and Care

After the successful deprogramming phase, we prepared the patient’s teeth for porcelain veneers. This involved removing approximately 0.5mm of tooth structure to ensure a seamless transition from the veneers to the natural tooth at the margins. The minimal removal of tooth structure is essential for the longevity and aesthetic appeal of the veneers.

The Final Steps: Crafting and Placing the Restorations

In collaboration with RDT Dental Lab, we created the porcelain veneers and dental crowns. The lab’s expertise ensured that the restorations were of the highest quality, matching the patient’s desired shade and natural appearance.

During the first try-in session, the patient was ecstatic with the results. The veneers and crowns fit perfectly, enhancing his smile while maintaining a natural look. He gave us his consent for the final placement, and we proceeded with the insertion of the restorations.

The Result: A Transformed Smile and Renewed Confidence

The final outcome was nothing short of spectacular. By adjusting the vertical dimension of occlusion and addressing the length of the anterior teeth, we not only improved the aesthetics but also the functionality of the patient’s smile. The traumatic occlusion was resolved, preventing further damage and ensuring the longevity of his teeth.

The patient was overjoyed with the transformation. His new smile was everything he had hoped for – natural, beautiful, and perfectly balanced. The veneers were not overly white, but a shade that complemented his complexion and looked incredibly natural.
